HARBOR SPRINGS — The Personal Action Toward Health workshop (PATH) will be offered from 1-3:30 p.m. starting Wednesday, Jan. 14, through Feb. 18 at the Village of Hillside Senior Center, 311 West Main St. in Harbor Springs.
This six-week chronic disease self-management program teaches knowledge and skills needed to better manage chronic diseases and has been proven in studies to reduce symptoms and physician visits, as well as increase self-management abilities and communication with physicians.
Developed at Stanford University, the program is taught by Stanford Chronic Disease Self-Management Program certified instructors. Each leader has completed in-depth training on the program. At least one leader lives with and manages a chronic disease.
“Persons with chronic disease such as diabetes, arthritis, high blood pressure, COPD, (and others), will learn to manage the disease they have by attending this six-week program,” said Judy Fowler, RN, PATH instructor and education specialist at the Northern Michigan Regional Hospital Community Health Education Center.
Through informal, small group discussion and easy-to-understand course materials, participants learn practical tips and techniques to take control of their condition. Classes address ways to reduce pain and stress, cope with fatigue, use medications wisely, and benefit from exercise. Participants also learn about self-help devices that can enhance daily activities and are offered an opportunity to seek answers to questions.
The program is suited for both the newly diagnosed as well as those seeking more aggressive management. Attending all six sessions is recommended. Spouses and caregivers are also encouraged to attend in order to better understand and live with the condition their loved one is faced with.
